In simple terms, Fibromyalgia actually means pain throughout the body. It is believed to be a disorder in the way the nervous system processes pain, together with a genetic disposition and a variety of environmental triggers that make someone with the right propensities succumb to the syndrome. Chronic widespread musculoskeletal pain is also a primary feature of the disorder. The fatigue of a sleepless night is drained into it every night, night after night.

People with fibromyalgia often wake up tired and heavy, with difficulty concentrating and being active. Experts believe this is caused by a combination of many factors. These include genetics, infections, physical or emotional trauma, and the effect of the disease. Many people who suffer from fibromyalgia also suffer from other conditions, such as irritable bowel syndrome or migraine. Scientists believe that certain versions of genes can make you more susceptible to fibromyalgia.

The disorder can be difficult to diagnose, as many fibromyalgia symptoms are similar to those of other conditions. Your doctor will assess the severity and length of widespread pain for at least three months.

You must undergo assessments for other conditions before a diagnosis is confirmed, for instance, by ruling out other explanations and assessing the quantity of tender areas in your body. These are specific locations that feel tender or hurt when slight pressure is applied to the muscle, for example. When areas of the muscles under pressure are more painful because of fibromyalgia, the disease process is triggered by the physical characteristics of the abnormal pain. If these points are located in at least 11 tender areas during a test in a specific, consistent manner, a repetitive or generalized pain is thought to occur.

Investigating the Causes of Fibromyalgia

Fibromyalgia (FM), a syndrome defined as chronic, widespread musculoskeletal pain and characterized by diffuse tenderness, fatigue, and multiple other somatic symptoms, is a common syndrome whose pathophysiology is incompletely understood. FM is clearly heterogeneous, making it important to identify its contributing phenotypic dimensions.

Disparities between patients and controls or between genetic polymorphisms and disease features might help identify patterns, mechanisms, and clinical structures of pain. Investigation of such disparities might indicate depressive disorders or degrees of obesity. Indeed, evidence has shown that FM is characterized by both comorbid psychosomatic traits and an associated genetic background.

Therefore, the two branches of current biological research in FM, namely genetic and epigenetic research, can be considered as pathophysiological endpoints of a common origin, since they investigate symptoms resulting from a functional imbalance between endorphinergic and anti-endophinergic systems.

In this comprehensive literature review, our first aim was to identify recent case-control studies demonstrating dissimilarities among FM patients, specifically showing whether there are significant discrepancies between patients’ genotype or allelic frequencies and either their depressive level or their degree of obesity and the severity of FM. We understand that elucidating the importance of these disparity determinants could assist clinicians or nurses in facilitating healthcare prevention programs for specific at-risk patients. Second, our aim was to systematize recent genetic association studies addressing the expected pathophysiological underpinnings of FM, thereby providing a wealth of new avenues for ongoing future FM research with which to reconcile positive clinical FM outcomes.

The third research aim was to clarify whether some inter-patient endophenotypic variability might underlie the apparent inconclusiveness of results. Such variability may arise from gene-environment interactions or from the involvement of some epigenetic regulators of these complex traits.

Our results highlight the FM features associated with each polymorphism against the patients’ normality, whereas our conclusion offers an up-to-date state of contemporary FM genetic research and sets these findings in the more general context of investigation.

Definition and Symptoms

Fibromyalgia, a muscular and nervous affliction, manifests itself with symptoms such as widespread subcutaneous pain, fatigue, and abnormal sleep patterns, often caused by several reasons. Especially, stress and depression are claimed to be important contributing factors. In the early stages of the disease, the patient frequently feels tired, experiences pain in certain positions, and endures general discomfort. Essentially, dysmenorrhea like complaints are present.

When an unfavorable period is experienced in the future, the patient feels more tired, and it lasts longer. Gradually, the feeling of discomfort is perceived in many muscles and organs in the body, making it necessary to consult a physician. Patients cannot feel good when they get up in the morning after nights in which they feel pain almost everywhere. At that time, a diagnosis is mostly made, and life will never be the same again as it used to be. People with fibromyalgia feel pain almost in every muscle of their bodies permanently. Especially, the muscle-specific points, known as “tender points,” which may be 11 or 18 throughout the body, are more painful.

These points are more painful even with a little pressure. However, that pressure does not require significant force. Therefore, the disease is one of the most painful conditions in medicine. In fibromyalgia, muscle pain is widespread and acute. It is possible to use muscle stiffness as another defining characteristic of this disease because it makes the performance of the muscles difficult.

When the illness is first recognized and becomes definite, there is no specific marker found for the diagnosis. Fibromyalgia is diagnosed serologically. The problem originates from the hormone called cortisol, which is anti-inflammatory, and the situation arises from a failure to pay attention to fatigue and sleep disturbances.
